THE CHALLENGE

The Problem We Were Asked to Solve

A network of 12 private medical clinics was running their appointment and patient communication system on a combination of phone calls, paper forms, and a 2009-era web portal that only worked on Internet Explorer. Patients were frustrated. Staff were overwhelmed with phone calls. No-show rates were running at 22%, with each one costing the network approximately $180 in lost appointment revenue.

The practice manager had evaluated three enterprise healthcare software vendors. Each quoted between $180,000 and $320,000 per year in licensing, before implementation costs. None could confirm HIPAA compliance for their mobile offering. One required the clinics to migrate their entire EHR system to get the mobile features.

They needed a custom mobile app that integrated with their existing EHR (a self-hosted, slightly customized version of OpenEMR), handled HIPAA requirements, and didn't require staff to change their core workflow.

THE SOLUTION

How We Solved It

We proposed a Flutter-based cross-platform mobile app (iOS + Android) with a custom Node.js backend, designed for HIPAA compliance from day one, not as an afterthought.

HIPAA Architecture Decisions:

  • All PHI encrypted at rest (AES-256) and in transit (TLS 1.3 minimum)
  • Business Associate Agreement (BAA) in place with all cloud providers
  • Zero PHI stored in app logs, crash reports, or analytics events
  • Session timeouts and biometric lock for app access
  • Audit trail for all data access and modifications

Core App Features Built (in priority order):

  1. Appointment booking: see real-time availability, book, reschedule, cancel
  2. Automated reminders: configurable SMS + push notifications at 48h and 2h
  3. Digital intake forms: complete pre-visit paperwork in the app (replaces paper forms)
  4. Secure messaging: HIPAA-compliant patient-to-clinic messaging
  5. Visit history and results (read-only view of relevant EHR data)

EHR Integration: We built a custom integration layer between the app backend and the clinic's OpenEMR instance, using OpenEMR's FHIR API. This allowed real-time appointment availability and two-way appointment management without modifying the EHR database directly.
